RB &CC Ranger Transport Ltd, trading as Machinery Specialists was fined $50,000 and ordered to pay reparations of $70,000 under s6 of the HSE Act, after a man was killed by falling crates. A shipping container had 16 crates stacked in four-by-four columns. They were initially individually removed by a forkhoist but the method was changed to remove them three at a time. One of the columns looked askew and as the victim went into the container to investigate it column tilted and collapsed, trapping him under two crates weighing close to three tonnes. The DoL said the company should have identified the hazards and required employees to unload all the containers one at a time, and limit employee access ( Manukau DC, March 31 2010).
