Inter-Weave Ltd was fined $25,000 and ordered to pay reparations of $15,080 under s6 of the HSE Act when an employee’s hand and four fingers were crushed in rollers he was attempting to clean. He had reached around the side of the machine to access the on/off switch but could not reach it. The DoL said at the time of the accident the inward running nip of the rollers, exerting 2000kg of pressure, were not guarded. The only form of protection was a safety wire to stop the rollers. The company should have installed a guard to prevent access to the inward running nip or a light beam or laser curtain to automatically switch the machine off when tripped ( Auckland DC, June 18).
