EXTRACT SOLUTIONS LTD was fined $8000 under s6 and ordered to pay $7500 in reparations to an employee who suffered burns to her face, hand and neck when ethanol vapour ignited as a tank was being cleaned. The tank had held resin which was solid at room temperature. Another worker had used a heat gun to liquefy the resin and drain the tank, before filling it with ethanol to remove the residue. The heat gun was left on top of the tank, still connected to a power source. When the injured woman began to scrub inside the tank, the gun fell and she grabbed for it, knocking the on-switch. It started, igniting the ethanol vapour. The defendant company had been told of a better means of heating the resin, but had not implemented it (Nelson DC, March 23, 2005).
