LIGNOTECH DEVELOPMENTS LTD was fined $10,000 on each of two charges under s6 and ordered to pay a total of $17,500 in reparations after two employees were injured when they fell from the tines of a forklift. The injured men had been asked to clean the factory roof and arranged for a co-worker to lift them and a water blaster up to roof level – a height of 4.9m. As the prepared to climb onto the roof, the box they were standing in slipped from the forklift, throwing them and the water blaster to the ground. One of the men suffered fractures to both legs, his left knee, right hip and wrist and a head wound, while the other suffered head and back injuries. The men had received no health and safety training, no hazard assessment had been done in relation to the roof work, and there was no work platform for the forklift, despite the operator’s request for one ( Ashburton DC, September 26).
