HUHTAMAKI HENDERSON LTD was fined $10,000 under s6 and ordered to pay $25,000 in reparations after an employee’s fingers were amputated when he attempted to clear a blockage in a blower pipe that removed plastic off-cuts from a trim press. 
The injured man had returned from a break to be told of the blockage by his supervisor. The supervisor then left for a break and, noticing that there was no air coming from the blower, the man reached into the pipe in the belief that his supervisor had turned the machine off. The rotating blades of the blower amputated four fingers at the first knuckle and cut and dislocated his thumb ( Waitakere DC, December 16).
