Optimum Cleaning Systems Ltd was fined $5000 under s.6 and ordered to pay $4000 in reparations after an employee suffered a compound fracture and nerve damage when her arm was caught in an offal auger. The defendant company was engaged to clean the premises and machinery at a meatworks. Procedures had not been implemented to ensure equipment was isolated before it was cleaned. The injured woman had inserted her whole arm into the auger, attempting to retrieve material that was blocking the drain hole (Ashburton DC, September 13).
