OFF-ROAD ADVENTURES QUEENSTOWN LTD was fined $1000 under s25(3)(a) for failing to inform OSH of a serious harm incident that occurred in a place or work under its control. The incident, on November 22 2003, involved a paying customer who fell during an off-road ATV tour. She was not initially thought to be seriously injured, but was subsequently found to have fractures to her arm, pelvis and hip and lacerations to her liver. OSH was not notified of the event until the end of the month (Queenstown DC, November 1).
