CMI LTD was fined $3000 under s.6 of the Health and Safety in Employment Act after an employee lost part of two fingers in a punch and forming press while setting up the tool and die. An electrician had replaced the solenoid with a different type, which negated the electrical interlock two-button control. A failure of communication allowed the machine to be put back into action before maintenance was complete. (Auckland DC, 25 February)
