Poultry processor SANTA ROSA MARKETING LTD was fined $7000 under s.6 and $1000 under s.7 after an employee's hand was crushed in a chicken pattie forming machine while cleaning the inside of a hopper. His supervisor had used a toggle switch to isolate the hopper feed rams but had not turned off the machine. His loose-fitting plastic sleeve covering caught the isolation switch and operated it, causing one of the rams to come down. No hazard-id had been carried out on the machine, and isolation procedures were inadequate. The injured man, three of whose fingers were crushed and dislocated and a fourth partially amputated, was awarded $5600. (Christchurch DC, 30 May)
