PACIFICA SHIPPING (1985) LTD was fined $10,000 under s6 and $2000 under s12 after an employee, who was riding on a cargo lift, had his foot crushed between the ascending lift and the deck of a vessel in dry dock. The trapping hazard had not been idenfitied, nor had he been told that no one was to ride on the lift. Signs to that effect had been removed or painted over. Also, the lift controls had been over-ridden so that only one operator on one deck was required to operate it. The victim, who lost toes and part of his foot, was awarded $5000.
