Engineering and steel fabrication company A F MARTIN LTD was fined $12,000 under s6 after an employee’s foot was trapped in the plate rollers of a machine used to make fuel tank cylinders. The injured employee climbed onto the machine to enable a new cylinder to be moved, unaware that a colleague had climbed on to the machine at the other end of the cylinder. This second employee inadvertently activated the control lever and started the rollers moving. The start-up mechanism was not isolated at the main power isolation switch. The injured man, who lost most of the toes on his left foot, was awarded $10,000. (Palmerston North DC, 7 July 1999)
