WHITESTONE ROADING was fined $10,000 under s6 after an employee’s arm was trapped and crushed in an unguarded conveyor on a mobile gravel crushing plant while he was trying to free it up after a night’s frost. Despite missing his scheduled radio reporting time back to base - a precaution for solo workers - no response was forthcoming, and he was not discovered for two hours.
