Lack of written procedures and training in confined spaces cost SOUTHLAND DAIRY CO-OP LTD $7500 on a s6 charge after an employee lost consciousness while working on a pregassing machine. He was pulled from an oxygen-deficient manhole by a colleague. The company had no formal confined space entry procedure; a draft procedure had not been implemented. A multi-gas detector had been approved but had not been ordered. Nor was a safety harness provided. The victim, who recovered fully, was awarded $3000 of the fine. (Invercargill DC, 16 June)
