Packaging company CARTER HOLT HARVEY LTD was fined a total of $20,000 after an employee lost two fingers and had his hand degloved while clearing a blockage in a new machine still under commissioning. Charges under s7 (failure to identify new hazards) and s13 (failure to train) attracted fines of $8000 each, while a s6 charge saw a fine of $4000 imposed. The company failed to carry out a hazard identification exercise on the new machine, leaving the task to the overseas engineers commissioning it. No procedure for handling misfeeds had been demonstrated, nor had employees been instructed to ensure both parts of the machine were turned off before attempting to clear a jam. The injured man, who was in hospital for more than five weeks, was awarded $16,000. (Christchurch DC, 19 March)
