Cleaning company P & O SERVICES (NZ) LTD was fined $6000 under s6 after an employee lost his right hand and most of his forearm in the blades of a bone crusher at a meat processing plant. He was awarded all of the fine. The man was employed to clean the bone crusher, which was activated by a switch in another room and by a key on the machine. He had simply been told to remove the key from the machine; he had not been told the machine could be started up if the switch in the other room was turned on. Nor had the defendant formally identified the hazards a cleaner faced. (Hamilton DC, 5 December)
