Failure to notify a serious harm accident as soon as possible, and interference with the scene of an accident, cost WAREHOUSE BUILDING SUPPLIES NZ LTD $2000 and $200 respectively under s25(3)(a) and s26(1) of the HSE Act. An employee lost two fingers to the first knuckle when they were caught in the lifting mechanism of a forkhoist. OSH was not informed until late the next day. An inspector arrived to find the forkhoist back in operation. (Henderson DC, 1 August)
