San Fong Fabric and Banner Printing Ltd was fined a total of $4500 under three sections of the HSE Act when an employee lost half his thumb in a printing machine. The company had no system for identifying hazards and the machine's rollers were unguarded. The injured worker, who had not been formally trained in the safe use of the machine, received half the fine. His thumb was surgically re-attached. (Otahuhu DC, 12 April)
