Failure to provide protection against a fall of more than three metres saw HIREPOOL AUCKLAND LTD fined $4500 on each of two charges under r21(2) of the HSE Regulations 1995. The charges arose when two employees were observed erecting scaffolding at a height of about eight metres. The men wore harnesses but there was nothing to which they could be attached. The company had been warned five days earlier of the need to provide suitable harnesses, (Otahuhu DC, 25 October)
