Ignorance of the requirement to notify all cases of serious harm as soon as possible, and in writing within seven days, saw Edward John Edwards, trading as Edwards & Co, fined $1000 under s25(3)(a) and $100 under s25(3)(b) of the HSE Act. An employee suffered a severe laceration to the middle finger of his right hand when it was cut by a circular saw, but more than two weeks elapsed before OSH was notified of the accident - by the injured man, not his employer.
