Police are still considering whether to lay criminal charges after a welder died when a tank exploded at the company's Wiri site last year. WorkSafe has already laid a total of 33 charges against the company and its director.

Geordie Grieve (t/a Geordie Grieve Builders)

Fined $15,000 and ordered to pay reparation of $48,592.43 after an employee suffered skull fractures and complex head injuries when he fell from a ladder. He had been dismantling a 2.8m-high balcony. As he came down the ladder it slipped forward. The ladder's rubber non-slip feet were worn out.

Transport (Waimate) Ltd

Fined $54,000 and ordered to pay reparation of $100,000 after an employee was fatally crushed at its quarry in March 2015. He was the sole operator at the quarry. His severely injured body was found in the quarry shed beneath unguarded rotating machinery. There was no process to ensure maintenance was carried out only when the machine was not running, and no way for the operator to stop the motor in an emergency. Nor had the company ensured that the victim held an appropriate quarry management qualification.


Pleaded guilty to a charge under the HSE Act after a hoist operator was seriously injured at its Manaia site in October 2015. Sentencing is expected in September.

Bus mechanic

A 68-year-old mechanic has been charged by Police with injuring by unlawful act after a bus carrying 31 tourists lost control, overturned and slid into a roadside barrier in the Otira Gorge. Investigators discovered the brakes were going to fail regardless of what action the bus driver took.
