Oil company Total UK Ltd has been fined £1.4m (NZ$3.26m) after a contract fitter received fatal burns during work beneath a distillation column. Another man escaped with minor burns.

SHP reports the incident occurred in June 2010 at the Lindsey Oil Refinery in Lincolnshire. The two men were required to open equipment beneath a distillation column which contained hot crude oil. The oil was released and ignited a short time later.

The company pleaded guilty to a breach of reg 4 of the Control of Major Accident Hazards Regulations 1999. The HSE inspector who laid the charge said that although the task the men were doing was simple, the high level of risk ought to have been identified.

As an operator in charge of a major hazard facility, he said, Total UK had fallen “far short” of the standard required.
