Major construction company Costain Limited has been fined £525,000 (just over NZ$1 million) after a man was fatally crushed when his telehandler overturned while he tried to raise a pallet of tiles onto a fourth story roof in Newbury.

The September edition of SHP reports the July 2011 incident occurred when the telehandler's boom was fully raised but not extended, reducing its stability when being turned. The victim was forced to operate this way because the space between buildings was almost the same length as the telehandler with the boom lowered.

The Health and Safety Executive found the vehicle was not suitable for use in such a tight area, and that Costain had failed to assess the space constraints and devise a safe system of work. The company was also required to pay a further £90,000 in court costs.
